Reinstatement of Licensure
A licensee may reinstate a license which is expired for over one year by submitting a new application with the fee and all supporting documentation, as well as meeting the current licensing requirements. Once the application packet has been completed, the application will be forwarded to the Board for its review and action at its next regularly scheduled board meeting. Upon approval, your license will be reinstated.
